A joint effort between Missouri Department of Mental Health and Missouri Department of Social Services, this program identifies Medicaid participants with mental health or substance use disorders, high costs and high risk due to uncoordinated care, and those who are not receiving services from a behavioral health safety net.

Project Goals

  • Decrease early death of persons with chronic behavioral health and physical health conditions

  • Improve other health outcomes for participants (e.g. decrease their use of emergency rooms and inpatient hospital visits)

  • Reduce the total annual Medicaid cost per participant
